Notice of non-discrimination
Prime Time Smiles treats patients without regard to race, colour, national origin, age, disability or sex.
Help with communication
If you need help communicating with us, tell the front desk when you book and we will arrange what we can. That includes interpretation where a language other than English is easier for you, and it includes practical adjustments if a disability makes a standard appointment difficult.
Call (978) 770-2308 and say what you need. Doing that before the appointment rather than on the day gives us time to arrange it properly.
If you believe you have been treated unfairly
Raise it with the practice first. Ask for Kush Patel, DMD, Privacy Officer. You can call, write to us at 65A Drum Hill Road, Chelmsford, MA 01824, or say it at the desk.
You also have the right to complain directly to the U.S. Department of Health and Human Services Office for Civil Rights, and the practice will not retaliate against you for doing so.
This page is not legal advice. The practice should confirm its compliance contact and the language services it actually offers, and have this reviewed by counsel, before launch.
